GENERAL DESCRIPTION: As a member of the UHHS department and working as part of the Encampment Outreach Team, this position provides short-term (6-12 months) Case Management supports to adults who are homeless and facing complex challenges including health, housing, and substance use. Working from a harm reduction, trauma informed perspective, case managers provide individualized support through community-based and telephone contact to assist participants in identifying and achieving self-determined goals with a focus on shelter/housing consideration and access. Working closely with the client, our Streets to Homes partners, external service providers and community agencies, the case manager liaises with and coordinates access to additional internal TNG services as well external community service providers. This position reports to the Supervisor, TCAT Case Management

MAJOR DUTIES:

  • Build trust and rapport with people living in encampments via outreach and engagement
  • Conduct intake assessments with participants to identify needs and develop individualized case management plans to include the coordination of supports, referrals, and advocacy on health issues, substance use, mental health, housing etc.
  • Maintain ongoing support in the community through meeting clients in encampments, community, or TNG offices. Also provide accompaniments to appointments, and/or meetings in institutional settings etc.
  • Work collaboratively to develop and maintain working relationships with the Project’s Partners, Streets to Homes, and other external service providers including withdrawal management services, hospitals, law enforcement agencies, and housing providers
  • Support client engagement initiatives within the Agency including peer development initiatives
  • Intervene and deescalate crisis situations
  • Engage actively in Team Case Management, Clinical Consultation Meetings, Department and other meetings as directed.
  • Maintain accurate case management records and complete reporting required by funders and Supervisor, adhering to all PHIPA and TNG policies

REQUIREMENTS & QUALIFICATIONS:

  • BA/B.S.W plus 2 years of experience in human services, preferably as a case manager or a combined equivalent of education, lived experience, and/or significant related training
  • Minimum 2 years’ direct client experience working with individuals living with mental health, substance use issues, histories of trauma and homelessness
  • Experience, alignment and commitment to harm reduction principles and practice
  • Commitment to anti-racism and anti-oppressive practices
  • Formal training in substance use and mental health is an asset
  • Strong familiarity and commitment to trauma informed care
  • Physically able to travel consistently throughout the city on transit
  • A thorough understanding of the concerns and needs of homeless individuals and families living in encampments and the ability to communicate effectively with them
  • The ability to work with minimal supervision and the flexibility to solve problems creatively
  • Excellent written and verbal communication skills
  • The ability to function as a member of a team whose members are located at separate working sites
  • Valid certification in Non-Violent Crisis Intervention and First Aid/CPR or willingness to be trained.
  • Recent demonstrated knowledge and skills in assessment, crisis intervention and prevention, conflict resolution and negotiation skills
  • Proficient with MS Office, and web-based data entry for case management and time/labour.
  • Familiarity with TREAT and the administration of the OCAN is an asset
