Caritas School of Life strives to support individuals facing the challenges of addiction and mental health concurrent disorders through healing provided in a day and residential therapeutic community program.

The role of a Case Manager is to work with clients to achieve safe, realistic and reasonable goals by supporting all individuals in fostering the tools to take on the challenges associated with addiction and mental health concurrent disorders. The Case Manager is also expected to collaborate with the interagency team on a daily basis to help support the caseload. The purpose of the position is to help clients while in the bed-based program and outpatient programs.

Duties and Responsibilities:

•Initiates services to clients at the point of Inquiry to ensure that proper assessment and initial service response occurs.

•Understanding and utilizing the techniques of the Therapeutic Community model in relation to clients’ files and support services

.•Develops clinically sound case plans, using appropriate assessment tools, that are specific to the needs of clients and the objectives of the program, such as Gain Short Screener, Intake Assessment, and discharge planning for the caseload

.•Conduct a GAINSS

• Ensures that all administrative functions related to the client are completed, including medical needs while the client is in program

• Maintains accurate and complete clinical records to include daily and monthly data management using TREAT

• Provides leadership in ensuring case conferences occur as required, such as the facilitation of family meetings and subsequent verification logistics

•Facilitation of weekly inquiry session in a group format

• Establishes case management links with other resources within both Caritas and the community

• Pursues case management activities independently and in concert with other professionals, agencies, and groups

• Collaborates with other appropriate service systems, organizations, and individuals to ensure that services are well co-ordinated and meet a high standard of quality care

•Management of all relevant agency information systems (e.g. Connex/Datis/Catalyst/TREAT

Acting as a mentor and a role model by following core principles, demonstrating honesty, integrity, and willingness to listen to client issues

•Manage crisis situations and emergencies by taking appropriate steps and informing parties

•Report all client incidents (no matter how minors

•Manage Ontario Works process for clients (open accounts, manage cheques, close account

•Collaboration with designated pharmacy partners to ensure clients prescribed medications are obtained per the physicians

•Compile waiting list and provide ongoing contact/support to clients on the list

•Take part in team and management meetings for the purposes of ensuring coordinated and integrative programming, and resource sharing

•Be familiar with funder legislation and regulations, agency policies and procedures

•Preparing staff for Accreditation and ensuring compliance with standards of care

•Demonstrate strong teamwork and leadership skills within the interagency team

Key Skills and Abilities:

•Ability to support individuals who have complex mental health and/or addiction issues, history of trauma, violence/abuse, ect

•Understanding of mental health, addictions, health sector, and community resources

•Understanding and utilizing the techniques of the Therapeutic Community model in relation to addiction treatment.

•Knowledge of evidence informed practices, evidence-based treatment models, best practice approaches, Mental Health Act, related legislation, Mental Health recovery, anti-oppressive, trauma-informed and harm reduction principles.

•Knowledge and understanding of social determinants of health, and awareness of systemic barriers to service.

• Knowledge of and sensitivity to issues of gender, race, sexual orientation, and issues impacting diverse communities

•Sensitivity to, and ability to work with, a diverse staff and client group

.•Strong conflict resolution, negotiation, and crisis prevention/intervention skills.

•Excellent multi-tasking, manage competing priorities, and time management skills.

•Possess a culturally sensitive lens when working with clients and facilitating family & client meetings.

•Able to interact and communicate with others in an empathic and supportive manner while building effective therapeutic relations and rapport

.•A clear understanding of and ability to demonstrate professional ethics, boundaries, and judgem

ent.

•Strong administrative and organizational skills.

•Excellent communication, interpersonal skills, observation, listening, organization, and time management skills.

•Ability to exercise excellent judgment, flexibility, creativity, and sensitivity to handing situations and needs

.•Strong computer proficiency and familiarity client data management systems, ie. TREAT

Qualifications:

•Bachelor of Social Work or a degree in a relevant health-related discipline, with a considerable knowledge of substance abuse issues

.•In good standing with an accredited regulated professional associ

ation.

•Minimum of 2 years related experience in direct service delivery in the mental health and addiction

field.

•Significant knowledge and experience in varieties of counseling models including Solution-Focused Therapy, CBT, DBT and Motivational Interviewing

.•Knowledge and experience in harm reduction strategies.

•Excellent organizational and administrative skills.

•The ability to work independently

.•A working knowledge of data management systems preferably TREAT.

•Demonstrated experience supporting individuals with mental health and substance use issues from a recovery and harm reduction approach.

Knowledge and understanding of the client population and an ability to work effectively with people living with concurrent disorders.

• Excellent clinical judgement and decision-making skills including a keen awareness of participant-clinician boundary issues

.• Excellent knowledge of community resources and/or demonstrated ability to search for applicable resources/information.

•Fluency in a second language (French), this is a strong asset and highly preferred.

•A successful Police Vulnerable Sector Check, as per agency policy and procedures.

Schedule: Day shift with the flexibility to work one evening shift a week (1 pm- 9 pm)

License/Certifications:

• G Class Driver’s License and a personal insured vehicle

•ASIST training an asset

•GAIN3 Certification an asset
