Addiction and Mental Health Worker, Hubs

Direct Service - (OPSEU)
Full-Time, Contract (End date: March 31, 2028)
London, Ontario (London Zone)

At this time, we are hiring 6, Full-Time, Contract positions

The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access safety, basic needs, health care and other mental health and addiction services under one location.

WHAT WE'RE OFFERING:

  • What you'll be making -Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
  • Successful candidates who are registered with one of the designated professional colleges (e.g., OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation.
  • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated).
  • For internal applicants, grid level placement will occur as per the Collective Agreement.

When you'll work- 70 hours over a two-week period; includes Days/Afternoons/Evenings/Nights including weekends.

Your pension plan– Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP).

HOW YOU WILL MAKE A DIFFERENCE:

  • Provide a welcoming, safe, and trusting environment for vulnerable individuals through intake, screening, assessment, and treatment planning, income and employment support, and supportive housing.
  • Support clients on their journey towards physical/mental health improvement, providing integrated care plans that encompass acute and primary care, mental health, addictions, income, transportation, and safety.
  • Ensure clients' basic needs are met (e.g., food, showers, hygiene, rest).
  • Provide referrals to other supports and resources to maintain overall wellness.
  • Assist with development of care plans, navigating local services, and engaging in proactive crisis intervention.
  • Provide individual and group counselling sessions with a strengths-based wellness approach.
  • Assist with navigation, connecting clients to resources, and collaborating with justice supports for those involved with the criminal justice system (pre-release, probation, bail).
  • Assist with transportation needs at intake, including obtaining travel assistance such as bus passes or other forms of travel, leveraging existing funding support.
  • Provide accessible communication options for clients with varying needs and literacy levels (AODA, multiple languages, sign language, audio translation, language classes referral).
  • Update client records according to organization standards, discipline-specific guidelines, and PHIPA legislation, handling in accordance with privacy laws and policies.
  • Provide support for additional HUB sites as required.

WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

Successful candidates must have a Police Vulnerable Sector Check completed within the last six months. Applicants residing in the City of London are encouraged to apply for one promptly, as PVSC processing takes approximately two weeks, and offers cannot be made without it.

Required qualifications:

  • Post-secondary education in a related field. Original degree/diploma/certificate or official transcript required during hiring; unofficial transcript accepted for ongoing education.
  • Valid driver's license and access to a vehicle with adequate insurance for job duties/transporting clients.
  • Ability to attend work regularly.

Preferred qualifications:

  • Current certification in Non-Violent Crisis Intervention (NVCI), ASIST, First Aid, CPR, CPI, etc.
  • Experience working with individuals with addictions, mental illness, homelessness, or related issues.
  • Experience in crisis intervention, community housing, or working with individuals with disabilities.
  • Bilingual in both official languages is preferred.
