The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs support clients through access to high-quality, evidence-informed care that is respectful, trauma-informed, culturally appropriate, and compassionate. It connects individuals with a continuum of care tailored to their needs, providing access to transitional supportive housing where individuals can access safety, basic needs, healthcare, and other mental health and addiction services in one location.

WHAT WE'RE OFFERING:
  1. Starting rate: $27.14 per hour (Level 1 – Direct Service), with a wage differential based on regulation ($30.01 per hour for regulated Level 1). Successful candidates registered with designated professional colleges will receive the wage differential. External applicants start at Level 1 with grid progression up to Level 8 ($34.41 per hour non-regulated, $37.26 regulated). Internal applicants' placement follows the Collective Agreement.
  2. Work schedule: Relief shifts including days, afternoons, evenings, nights, and weekends.
  3. Pension plan: Immediately eligible for optional enrollment in HOOPP.
HOW YOU WILL MAKE A DIFFERENCE:
  • Provide a welcoming, safe, and trusting environment for vulnerable individuals through intake, screening, assessment, and treatment planning.
  • Support clients on their journey towards health improvement with integrated care plans covering various needs.
  • Ensure basic needs are met and provide referrals to maintain overall wellness.
  • Assist with developing care plans, navigating local services, and engaging in crisis intervention.
  • Provide individual and group counseling with a strengths-based wellness approach.
  • Assist with transportation needs and facilitate communication options for clients with varying needs.
  • Maintain client records in accordance with standards, privacy laws, and policies.
  • Support additional HUB sites as required.
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements

Must have a recent PVSC. Applicants residing in London are encouraged to apply for one promptly.

Required Qualifications:
  • Post-secondary education in a related field with proof of credentials.
  • Valid driver’s license and access to a vehicle.
  • Ability to attend work regularly.
Preferred Qualifications:
  • Experience with addiction, mental illness, homelessness, crisis intervention, and community resources.
  • Certifications such as First Aid & CPR, ASIST, CPI.
  • Bilingual in both official languages preferred.
