Singapore Christian Home is seeking a qualified Social Worker to provide professional, ethical, and person-centred social work services in a long-term care setting. The role focuses on assessments, case management, and multidisciplinary collaboration to promote dignity, independence and quality of life for residents.

Responsibilities include psychosocial interventions, family support, admission coordination and timely referrals to healthcare and community resources.

Qualificações

  • Hold a recognized Bachelor's Degree, Graduate Diploma, or Master's in Social Work from SWAAB-accredited institution.
  • Be a registered social worker with the SWAAB.
  • Member of the SASW.

Responsabilidades

  • Conduct comprehensive social, psychosocial and BPSS assessments to identify needs and support systems.
  • Develop, implement and review individualized case management plans and interventions.
  • Provide psychosocial, therapeutic and supportive interventions for aging, illness and functional changes.
  • Offer counselling and information to residents, family members, caregivers and others.
  • Conduct ward rounds to identify psychosocial needs and coordinate follow-up.

About the role

The Social Worker provides professional, ethical and person-centred social work services to residents and their families in a long-term care environment. The role encompasses comprehensive social work and case management, admission and discharge planning, resident and family support, financial assistance coordination, and multidisciplinary collaboration to promote residents' dignity, independence, social integration and quality of life.

Key responsibilities
  • Conduct comprehensive social, psychosocial and Bio-Psychosocial-Spiritual (BPSS) assessments to identify the needs, strengths, risks and support systems of residents and their families

  • Develop, implement and review individualised case management plans and appropriate interventions based on residents' assessed needs

  • Provide appropriate psychosocial, therapeutic and supportive interventions to assist residents in coping with ageing, illness, changes in functional status and adjustment to the long-term care environment

  • Provide counselling, emotional support and relevant information to residents, family members, caregivers and significant others

  • Conduct regular interactions and ward rounds to identify residents' psychosocial needs and facilitate timely intervention and follow-up

  • Engage residents and their families proactively to promote family involvement, social integration, dignity, independence and quality of life

  • Conduct family conferences, case conferences, mitigation sessions and other appropriate interventions to address concerns, conflicts, complaints or negative feedback

  • Facilitate timely referrals to appropriate healthcare, social service and community resources based on residents' assessed needs

  • Maintain accurate, timely and confidential documentation of assessments, case management plans, interventions, referrals and progress for continuity of care, follow-up and audit purposes

  • Participate in the pre-admission screening and assessment of prospective residents and coordinate social work-related admission matters

About you
  • Hold a recognized Bachelor's Degree, Graduate Diploma, or Master's in Social Work from a Social Work Accreditation and Advisory Board (SWAAB)-accredited institution

  • Be a registered social worker with the SWAAB

  • Member of the Singapore Association of Social Workers (SASW)

  • At least two years of experience as a Social Worker in a Restructured/Community Hospital or Nursing Home

  • Ability to converse in local languages will be an added advantage for effective communication with residents and their next-of-kin

  • Keen to work in eldercare sector

  • Team player with good interpersonal, organisational and communication skills
