Job summary

Redcliffe Area Youth Space (RAYS) is seeking a Case Worker to provide intensive, relationship-based case management and outreach support for young people across the Moreton Bay region, including Redcliffe. You will work flexibly in homes, schools, and community settings to support safety, stability and growth.

You will collaborate with families, carers and partner services, undertake individualised planning and deliver practical interventions to improve outcomes.

Qualifications

• Experience working directly with young people with complex circumstances.
• Strong relationship-building and engagement with youth who may have disengaged.
• Experience in assessment, planning, practical intervention and coordinated support.

Responsibilities

• Build trusting, consistent relationships with young people and maintain connection.
• Undertake assessments and develop practical support plans with young people.
• Support understanding and navigation of bail conditions and practical barriers.
• Deliver interventions to strengthen emotional regulation, problem-solving and relationships.
• Provide practical outreach support including housing, education, health and other needs.
• Collaborate with families, carers and community services for coordinated care.
• Monitor risks and participate in case reviews with accurate records.
• Contribute to reflective practice and team learning.
