North West Queensland Indigenous Community Social Services Ltd. seeks a self-motivated Case Worker to join the Youth Transitional Hub, providing individualised case management for at-risk young people within a strengths-based, culturally responsive framework.

The Youth Transitional Hub provides culturally responsive support to at-risk young people through case management, diversion activities, outreach support, and safe overnight accommodation.
The program supports young people by providing a safe and supportive environment where they can build positive connections, develop life skills, access appropriate services, and receive guidance to improve their wellbeing and future opportunities. Through strong partnerships with families, community organisations, schools, health services, and justice agencies, the program aims to reduce engagement with anti-social behaviours and support young people to make positive choices.
NWQICSS is seeking a self-motivated and experienced Case Worker to join the Youth Transitional Hub team.
The Case Worker will provide individualised case management and support to young people, working within a strengths-based and culturally responsive framework. The role will involve identifying individual needs, developing case management plans, coordinating referrals and supporting young people to access services that promote positive outcomes.
A key component of the role will be working collaboratively with Youth Justice, QPS and other relevant agencies to assess the risks and needs of young people who may be engaging in anti-social behaviour or coming to the attention of police.
The successful applicant will demonstrate:
A Diploma-level qualification in Social Work, Child, Youth and Family Intervention or a related field. Applicants currently undertaking a relevant qualification may also be considered.
Demonstrated understanding of Aboriginal and Torres Strait Islander peoples, cultures, communities and protocols, including the contemporary issues impacting First Nations young people and families.
Demonstrated experience in case work and case management, preferably working with young people or vulnerable individuals.
Knowledge of relevant services and referral pathways available to young people and families, including accommodation, counselling, family support, legal, education, employment and alcohol and other drug services.
Demonstrated ability to develop and maintain effective working relationships with young people, families, service providers and other stakeholders.
Experience working collaboratively with government agencies, community organisations and other service providers.
A sound understanding of the role and responsibilities of a Case Worker.
Strong communication and interpersonal skills, with the ability to communicate effectively with young people, families and professional stakeholders.
Good written communication skills and the ability to complete case notes, reports and other documentation accurately and within required timeframes.
Sound organisational and time management skills, with the ability to manage competing priorities.
Good computer skills and confidence using electronic systems and Microsoft Office applications.
A willingness to undertake relevant training and professional development.
A current Australian Driver Licence.
A current Blue Card or ability to obtain one.
A current National Police Check or ability to obtain one.
