Berry Street is seeking a dedicated Case Manager for the Multi-Agency Support Team (MAST) in Ballarat. You will work with the Senior MAST Coordinator and partner agencies to coordinate practical, individualized support for young people aged 10–17, across Ballarat, Moorabool and Ararat.

You will help develop plans, strengthen protective factors and connect youth with education, family and community services.

Qualifications

• Experience in case management with young people and families.
• Strong communication, advocacy and stakeholder engagement skills.
• Understanding of risk and protective factors related to youth offending.
• Experience with diverse backgrounds and complex needs.

Responsabilités

• Coordinate practical, individualized support for young people and families.
• Contribute to Action and Support Plans to strengthen protective factors.
• Collaborate with police, education and family services to coordinate services.
