SERO4 Ltd's MARA Project in Rockhampton, Central Queensland, seeks a compassionate Community Reintegration Worker to support women transitioning from custody back into the community. You will provide strengths-based case management, coordinate housing, health and social supports, collaborate with Corrective Services and stakeholders, and help women build independence and positive community connections.

Qualifikationen

• Experience in community services or related field.
• Understanding trauma-informed, strengths-based and person-centred practice.
• Experience in case management, advocacy or community-based support.
• Strong communication skills with women, colleagues and stakeholders.

Aufgaben

• Provide individualised, strengths-based case management to women transitioning from custody into the community.
• Build supportive, professional relationships with women, recognising their experiences and goals.
• Support women to access housing, healthcare, mental health and AOD services, education, employment and other supports.
• Collaborate with Queensland Corrective Services and stakeholders to support reintegration outcomes.
• Provide practical and emotional support that encourages independence and resilience.
• Advocate alongside women to help navigate services and barriers.
• Help women develop and work towards their Path to Change.
• Maintain accurate case notes, reporting and data entry per organisational requirements.
• Travel throughout the service region to meet with women and stakeholders.
