Job summary

Marist180 is seeking a Caseworker based in Western Sydney to provide comprehensive case management across residential and outreach programs. The role emphasizes trauma-informed practice, development of client-focused plans, and collaboration with care teams to support safety, permanency and wellbeing for young people.

You will develop pathways for permanency, coordinate services, and engage families and professionals to achieve positive outcomes for clients in alignment with agency standards.

Qualifications

• 2 years' experience in case management.
• Experience working with clients (ages 12-18) who have experienced complex trauma.
• Experience working with families involved with child protection systems.
• Understanding of residential or foster care contexts and child development.

Responsibilities

• Develop and maintain professional networks within the agency and community services to improve outcomes for our clients.
• Provide specialist case management advice to residential and support staff in planning, implementing and monitoring case plans.
• Coordinate and monitor support and services for clients in residential services and outreach programs within a trauma-informed framework.
• Undertake holistic assessment of clients including safety, permanency and wellbeing needs.
• Review case plans with clients and stakeholders on a quarterly basis.
• Build and maintain trusting therapeutic relationships with clients and their families.
