The Salvation Army ACT Central Intake Service is seeking an Intake Case Worker to deliver trauma-informed intake, assessment and short-term case coordination for individuals and families facing hardship. The role involves triage, multiple access points, and close collaboration with ACCOs to ensure culturally safe responses.

You will conduct holistic assessments, provide brief interventions, and refer clients to long-term supports with follow-up as required.

Qualifications

• Minimum Diploma in Community Services, Social Work, or related field.
• Experience delivering case work or community support acceptable.
• At least 1 year of case management or related experience.

Responsabilités

• Hholistic assessments using ACT Shared Practice Framework.
• Provide brief, goal-oriented interventions (safety planning, crisis support).
• Refer clients to long-term supports and follow up as needed.
• Record assessments and client data in SAMIS, SHIP or similar systems.
• Deliver intake services via phone, in-office, and outreach.
• Ensure cultural safety and accessibility across modes.
