Job summary

MacKillop Family Services is seeking a Home-Based Care Case Manager in Geelong to support children and young people in foster care. The role focuses on trauma-informed care, safety, stability and emotional well-being, with regular home visits and caregiver support.

The role is a full-time fixed-term backfill ending 08 January 2028, onsite in Geelong, reporting to the Home-Based Care Coordinator. Successful candidates will demonstrate cultural awareness and strong documentation practices.

Qualifications

• Tertiary qualifications in Social Work, Psychology, or related behavioural sciences.
• Strong communication and interpersonal skills, with the ability to work collaboratively in multidisciplinary teams.
• Experience providing case management in complex family situations within child protection.

Responsibilities

• Provide comprehensive home-based care case management and supervise caregivers.
• Conduct regular home visits and annual assessments for clients.
• Coordinate care plans with Child Protection and community services; ensure documentation and compliance.
