Job summary

MacKillop Family Services in Warrnambool is seeking a dedicated Case Manager for our Residential Care Program. You will work with young people, families and professionals to develop and review case plans, coordinate services, and advocate for needs.

This permanent part-time role is 3 days per week with negotiable hours. You will promote safety and wellbeing, maintain records, and contribute to a supportive team culture.

Qualifications

• Valid and current Driver’s Licence.
• Satisfactory Criminal History Check (paid for by MacKillop).
• Valid Employee Working with Children Check.
• Valid and current NDIS Workers Screening Check (MacKillop can assist successful candidates with their application).

Responsibilities

• Support children and young people living in residential care to achieve their goals and reach positive outcomes.
• Develop and review case plans that reflect the individual needs, strengths, and aspirations of young people.
• Coordinate services and advocate for young people to ensure they have access to the support they need.
• Build positive relationships with families, carers, community services, and other key stakeholders.
• Facilitate care team meetings and contribute to planning and decision-making processes.
• Maintain accurate records, reports, and case documentation.
