Job summary

Centacare Catholic Family Services in Mount Gambier is seeking a part-time Case Manager to support women and children transitioning from crisis care through therapeutic case management and group support.

You will work with a trauma-informed approach, conduct risk assessments and safety planning, and help clients access long-term community supports. Degree or Diploma in Social Work or related field and valid checks are required.

Qualifications

• Degree or Diploma in Social Work, Social Sciences, Community Services, or equivalent.
• DHS Working with Children Check required.
• Australian Driver's Licence required.
• Ability to work outside standard hours when required.

Responsibilities

• Provide therapeutic case management and group support to women and their children exiting crisis care.
• Conduct risk assessments and safety planning to build sustainable community connections.
• Apply trauma-informed practices to support resilience and economic independence.
