Job summary

Centacare Catholic Family Services in Mount Gambier seeks a Case Manager for the EASE program to deliver trauma-informed, early intervention case management to victim-survivors across the Limestone Coast. You will provide face-to-face, community-based support tailored to diverse backgrounds and circumstances.

You will empower clients to enhance safety and wellbeing through timely advocacy, connections to services, and collaborative work with the Team Leader and Community Development Worker.

Qualifications

• Degree or Diploma in Social Work, Social Sciences, Community Services, or equivalent experience commensurate with the position.
• Valid Department of Human Services (DHS) Working with Children Check
• Valid Australian Drivers Licence
• Ability to do some work outside of standard hours when required

Responsibilities

• Provide trauma-informed, strengths-based case management support to victim-survivors.
• Ensure victim-survivors are informed about the support options and services available to them.
• Connect individuals with meaningful community networks, resources, and support services that meet their needs.
• Support victim-survivors to develop and strengthen safety planning and risk management strategies.
• Work collaboratively with colleagues and community stakeholders to achieve program objectives and improve service outcomes.
• Maintain accurate case records and ensure service delivery aligns with program guidelines, organisational policies, and best practice standards.

Education

Degree or Diploma in Social Work, Social Sciences, Community Services, or equivalent experience commensurate with the position.
