Job summary

Me & My Support is seeking a skilled Full Time Supported Living Coordinator to join our team at the North Brisbane Office. You will coordinate day-to-day Supported Living and other Lifestyles Services for eligible NDIS participants, ensuring alignment with our service standards and practice framework.

The role requires leadership experience, rostering skills, NDIS knowledge, and the ability to work independently in a fast-paced environment. On-call duties are required as part of the position.

Qualifications

• Experience in a similar role is required.
• NDIS framework knowledge essential.
• Ability to lead and manage SIL homes.
• Strong rostering and scheduling capability.
• Proven written and verbal communication.

Responsibilities

• Coordinate day-to-day Supported Living and related services for eligible NDIS participants.
• Ensure service delivery aligns with MMS standards and practice framework.
• Support on-call duties as required.
• Maintain alignment with NDIS Price Guide & Practice Standards.

We are looking for a skilled and experienced Full Time Supported Living Coordinator to join our growing team to help us to continue to pursue our purpose of providing high quality, responsive supported living services that empower people with disabilities to live with autonomy, feel truly heard and experience a life of dignity, purpose and belonging. Position situated at our North Brisbane Office.

About the position

The Supported Living Coordinator plays an important role within the Operational Team and is responsible for coordinating day to day Supported Living & other Lifestyles Services offered to eligible NDIS Participant's. The Supported Living Coordinator is responsible for achieving the MMS service standard and operating effectively under the established practice framework and provisions.

ABOUT US

Me & My Support is a Contemporary Disability Support Service that operates as a Registered NDIS throughout Brisbane, Gold Coast & Sunshine Coast. Me & My Support specialises in providing Supported Independent Living Services funded by the National Disability Insurance Scheme.

NOTE: Must be willing to participate in OnCall duties

Experience, Attributes & Skills

• Experience in a similar role, compulsory
• Satisfactory hands-on leadership experience
• Experience coordinating supports for people with a disability under the NDIS framework
• Managing SIL Homes
• Rostering and scheduling experience, desirable
• Positive and contemporary attitude
• Ability to maintain professional development
• Good knowledge of NDIS Price Guide & NDIS Practice Standards
• Experience with Incident & Conflict Management
