Job summary

Claro Disability Services in Melbourne is seeking caring, person-centered support workers to assist clients with complex care needs. You will help with mobility, daily tasks, and social participation, tailoring supports to each client’s goals and culture.

Responsibilities

• Chat with the client (and family/advocates) to understand goals and routines
• Help with mobility, transfers, positioning, daily tasks and devices as outlined in the plan
• Watch for changes and liaise with the care team about med/clinical plans
• Provide social time and community access aligned with interests
• Support decision-making, respect consent, and Boost independence where possible

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
