Job summary

Claro Disability Services in Melbourne, VIC, seeks a compassionate Support Worker to assist clients with complex care needs in accommodation settings. You’ll deliver person-centered care, mobility support, daily tasks, and engaging activities, coordinating with families, medical professionals and a clinical team.

The role offers paid induction, flexible shifts, and travel between client shifts, with a team that values reliability, empathy, and collaboration.

Responsibilities

• Chat with client, family and advocates to understand goals and routines.
• Assist with mobility, transfers, daily tasks and devices per plan.
• Monitor medical changes, follow clinical plans, liaise with care team.
• Provide social time and community access aligned with interests.
• Support decision-making, respect consent and boost independence.
• Respect cultural backgrounds and beliefs in daily care.

What you'll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
