Job summary

Claro is seeking a caring Support Worker to assist people with complex needs in community settings around Keilor Downs, Brisbane. You will provide person-centered care, assist with mobility, daily tasks, medical supports, and meaningful social activities, working with families and medical professionals to ensure smooth, compassionate care.

Ideal candidates are warm, reliable, great communicators and adaptable to changing needs, with a willingness to travel between client shifts and collaborate

Responsibilities

• Provide person-centered planning by engaging with clients, families, and advocates to understand goals and routines.
• Deliver complex physical support including mobility, transfers, positioning and daily tasks.
• Follow medical/clinical plans and liaise with the care team for changes.
• Facilitate social participation and community access aligned with client interests.
• Support decision-making, maintain consent, and promote independence.
• Demonstrate cultural humility and respect for diverse backgrounds.

What you'll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

What are the hours?

• Monday 5pm-8pm
• Tuesday 4pm-8pm
• Wednesday 5pm-7:30pm
• Friday 5pm-8pm
