Job summary

Claro Disability Services in Victoria is seeking compassionate care workers to support clients with complex needs across our Frankston, Mornington, and Hastings sites. You’ll deliver person-centered care, assist with mobility and daily tasks, and collaborate with families and clinicians to ensure safety and wellbeing.

This role offers flexible shifts, paid induction and travel between client shifts, and real opportunities to make an impact.

Responsibilities

• Person-centered planning with client, family, and advocates to understand goals and routines
• Assist with mobility, transfers, positioning, daily tasks and devices per care plan
• Provide medical supports and liaise with care team for changes in condition
• Facilitate social and community participation aligned with client interests
• Support autonomy and advocacy, respect consent and independence
• Demonstrate cultural humility and respect for diverse backgrounds

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
