Job summary

West Wimmera Health Service is seeking casual Support Workers to provide in-home and community-based services across multiple locations in the West Wimmera region. The role supports independence and participation in community life, including TAC, NDIS and CHSP clients.

Essential duties include clear communication, person-centered planning and adherence to health and safety directives. Pre-requisites include screening clearances and a current First Aid Level 2 certificate.

Qualifications

• Current First Aid Certificate (Level 2) is essential.
• Desirable: Qualification in Aged Care, Disability, Individual Support, Community Services or equivalent.
• Desirable: previous experience in community or residential aged care or disability setting.

Responsibilities

• Provide support services for individuals in community-based supports, including NDIS participants and CHSP/SaH clients.
• Assist clients to achieve goals outlined in their Support Plan or Care Plan.
• Promote positive community attitudes related to disability and ageing.

West Wimmera Health Service provides high-quality hospital care, residential aged care, disability services and community health facilities to more than 22,000 people across nine locations in the West Wimmera region.

RENUMERATION

• Social Community Home Care & Disability Services Industry Award.
• Classification Range - Support Worker Employee HC2 - HC5 Level 2 to Level 5 (level is dependent on qualifications and experiences per Award).

ABOUT THE ROLE

• Provision of in-home and community-based supports which enable people to maintain and/or increase their independence.

KEY SELECTION CRITERIA

Essential

• Effective communication and organisational skills.
• Commitment to principles of empowerment, inclusion, choice and control when supporting people to maximize their independence and participate in community life.
• Current First Aid Certificate (Level 2) or willingness to obtain prior to commencement.

Desirable:

• Qualification (Certificate or above) in Aged Care, Disability, Individual Support, Community Services, or equivalent field.
• Previous experience working in a community or residential aged care or disability setting.

PRE-REQUISITES

• NDIS Worker Screening Clearance.
• Working with Children Check.
• Fully vaccinated (including influenza) as per current Department of Health Directives.
• Australian Driver's Licence.

KEY RESPONSIBILITIES

• To provide support services for individuals receiving community-based support, including Traffic Accidents Commission (TAC) clients, National Disability Insurance Scheme (NDIS) participants, and/or community aged care Commonwealth Home Support (CHSP) and Support at Home (SaH) clients.
• Assist clients/participants to achieve their individual goals, as outlined in their Support Plan or Care Plan.
• Promote positive community attitudes in relation to disability, ageing and inclusiveness
