Job summary

Action Justice Well-being is seeking a Disability Support Worker to provide direct assistance to clients per their service plans, enabling participation in age-appropriate activities, employment, in-home support and community access.

You will work with clients, families and professionals, ensuring safety, privacy and accurate record-keeping while contributing to planning and outcomes. A team-oriented role requiring strong communication and coordination skills.

Qualifications

• Current National Police Check
• Working with Children Check
• First Aid certificate and CPR
• NDIS Workers Orientation
• 100 Points of ID
• Minimum of 1 month relevant experience in a similar role
• Previous experience supporting people with disability
• Demonstrated proficient written, verbal and interpersonal communication skills
• Demonstrated ability to work with minimal supervision and collaboratively as a member of a multidisciplinary team
• Ability to effectively prioritise and organise own work and caseload

Responsibilities

• Support clients to participate in age appropriate activities and tasks including employment, in home support and community access
• Support clients to access activities and outings in the community
• Provide direct assistance to each client in accordance with their service plan and as directed
• Monitor the safety and well-being of the clients and report any concerns to the Manager
• Ensure completion of appropriate documentation and record relevant information on support provided in client notes and communication book as required
• Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes
• Work with clients, family/carers, relevant case managers and other professionals to support the client's individual goals
• Assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals
• Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress
• Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review

Job description
