About the role
Support coordinators play an important part in safeguarding a participant’s wellbeing. You’re well placed to spot concerns early about the quality or safety of someone’s supports, and you’ll help participants optimise their plan so they get the most out of their funded supports.
Working with participants
- Connect participants with relevant services and supports
- Help them understand their plan funding and make the most of it
- Negotiate service agreements and set up appointments
- Design support solutions and, where practical, an action plan
- Reduce barriers to using the plan as intended — including access to education, health or justice services
- Coach participants through challenges and help resolve points of crisis
- Help participants prepare for plan reviews and report on their outcomes
Building capacity
- Strengthen participants’ ability to design and direct their own supports
- Build connections with community and broader systems of support
- Help participants take charge of their lives and wider goals
- Work in a time-limited, outcomes-focused way with all stakeholders
Quality, safety and culture
- Model NDIS principles and values in everything you do
- Identify heightened risk and support workers to manage it
- Recognise and report signs of discrimination, exploitation, neglect, abuse or violence, and any unauthorised restrictive practice
- Make sure participants and workers know how to give feedback or make a complaint
- Support positive risk-taking with individual safeguards
- Champion diversity, inclusion and the voices of people with lived experience
Requirements
- Strong knowledge of the NDIS — especially support coordination and the importance of choice and control
- A cleared NDIS Worker Screening Check
- Completion of the NDIS Worker Orientation Module
- A qualification in social work, allied health or similar with disability experience is desirable, but not essential
Experience and skills
Experience (preferred)
- Working within NDIS principles, Practice Standards and related legislation
- Applying the UN Convention on the Rights of Persons with Disabilities in practice
- Current best-practice models in disability services, including the Positive Behaviour Support Capability Framework
- Supporting people who may be at heightened risk
Skills
- Coordinating services across complex, multi-disciplinary teams
- Preparing comprehensive reports with stakeholders
- Excellent communication, active listening and interpersonal skills
- Critical thinking, problem solving and organisation
- Strong time management and Microsoft Office skills
- A team player who builds rapport and trust
Together we can make a change.
