To oversee all NDIS relevant tasks for participants who seek support and guidance with their NDIS plan. You will be fully responsible for support coordination and other NDIS relevant tasks as they arise.
Key responsibilities
- Review and accept Support Coordination referrals as they present, following internal processes and policies
- Maintain full responsibility for the supervision of NDIS plans
- Undertake effective person-centred communication with clients, their supports and services under the duty of care
- Maintain case notes, seek service agreements, and pursue relevant reports
- Regularly review spending while working cohesively with the administration team and colleagues
- Keep abreast of all industry changes as the NDIS is a rapidly growing and changing scheme
- Liaise regularly with professionals in the industry and represent the company with professionalism at all times
- Participate in the risk management program and contribute to a clean, safe work environment
- Actively participate and contribute to quality activities which promote continuous improvement and adhere to ISO practices
- Actively participate in relevant professional development and requested training
