Job summary

Eunoia Lane in Cairns is seeking a Specialist Support Coordinator to support clients with complex care across the lifespan and to implement their NDIS plans. You will build trust with clients, families, and service providers, coordinating across clinical staff and government departments.

The role focuses on person-centred supports, collaborative stakeholder engagement, and timely service delivery, contributing to sustainable positive outcomes and ongoing staff development.

Qualifications

• A bachelor’s degree in social work or other relevant discipline (Occupational Therapy, Physiotherapy, Psychology, Nursing or other Allied Health);
• Demonstrated well-developed oral, written and interpersonal communication skills.
• Demonstrated ability to work well independently and cooperatively as a member of a team.
• Established ability to listen actively and demonstrate empathy.
• Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.
• Working with Children card; NDIS Worker Screening; Police check (within 6 months); current Drivers Licence and roadworthy vehicle.
• Computer literacy – efficiency in Microsoft Office.
• A willingness and ability to travel by car, 4WD, and/or light aircraft to rural and remote settings.
• Experience working with Aboriginal and Torres Strait Islander peoples with an interest in indigenous advancement.
• Experience working with Culturally and Linguistically Diverse (CALD) people and their support networks.

Responsibilities

• Identify and address barriers to implement clients’ NDIS plans across the life span.
• Build relationships with clients, families, disability services organisations, clinical staff and government departments.
• Collaborate with stakeholders to deliver supports in a person-centred and timely manner.
• Contribute to best practice care, staff training, and intervention for clients, including crisis periods.
