For Care is seeking an experienced Service Coordinator to support people with complex mental health and disability needs in a dynamic Brisbane/Redlands environment. The role focuses on onboarding, coordinating high-needs supports, and maintaining compassionate, person-centred service delivery.

You will build trust with participants, families and clinicians, manage escalations, and guide frontline teams to ensure consistent quality and safety in every interaction.

Qualifications

  • Proven experience coordinating complex mental health supports or similar high-needs community services.
  • Strong relationship management skills—build trust and maintain calm in challenging situations.
  • Ability to manage complexity without losing attention to detail or customer experience.
  • Operationally confident to onboard a cohort with minimal ramp-up time.
  • Coaching and mentoring team members and building capability.
  • Excellent judgement, emotional intelligence and communication skills.
  • Resilient, adaptable and solutions-focused.
  • Experience as a Service Coordinator within disability, mental health or human services.

Job description

For Care is an NDIS registered provider based in Capalaba, supporting participants across Redlands, Brisbane, Moreton Bay, Ipswich, Toowoomba, and the Gold Coast. We provide Supported Independent Living, in-home care, community access, personal care, transport and plan management services.

About the Role

We're looking for an experienced Service Coordinator to step into a live and growing mental health and disability support environment and provide steady, high-quality coordination from day one. This critical position, based in Brisbane/Redlands, will be supporting customers currently being onboarded into care — people with complex presentations, layered support systems, and networks that require consistency, confidence, and trust.

You’ll be joining at a pivotal stage, helping protect onboarding quality, relationship continuity, and the overall customer experience as services scale.

Reporting to our Operations Manager - Disability Services, you'll need to be someone who can walk into a moving environment, quickly build credibility, and confidently hold the operational and relational complexity that comes with high-needs mental health support — someone with a genuine understanding of mental health and the skill to manage complex behaviours calmly and safely, and who can bring the team along with them.

You’ll play a key role in:

Coordinating complex mental health supports for customers currently entering service

Understanding and responding to complex behaviours and mental health presentations using trauma-informed, person-centred approaches.

Managing onboarding continuity and ensuring participants feel safe, supported and understood

Building strong, steady relationships with participants, families, guardians, clinicians and broader support networks

Navigating escalations, service changes and competing priorities calmly and professionally

Working closely with frontline teams to ensure responsive, person-centred support delivery

Maintaining high-quality communication and follow-through in a fast-paced environment

About You

Proven experience coordinating complex mental health supports or similar high-needs community services

Strong relationship management skills — you know how to build trust and maintain calm in challenging situations

Navigating escalations, service changes and competing priorities calmly and professionally.

Someone operationally confident who can step into a live onboarding cohort without needing extensive ramp-up time

Coaching and mentoring team members, building their skills, confidence and capability in supporting participants with complex needs.

Excellent judgement, emotional intelligence and communication skills

Ability to manage complexity without losing attention to detail or customer experience

Resilient, adaptable and solutions-focused

Proven experience as a Service Coordinator within disability, mental health, community or human services
