Job summary

UnitingCare Wesley Bowden is seeking a Specialist Support Coordinator to provide person-centred support to people with high or complex needs under the NDIS framework. You will develop personalised plans, manage budgets, and build strong links with providers to enable safe, effective services.

You will guide participants on NDIS plans, respond to crises, and complete required NDIA reporting with integrity. A trauma‑informed, inclusive approach and strong organisational skills are essential.

Qualifications

• Compassionate, well-organised professional with experience in complex case management or specialist support coordination.
• Ability to support people with complex needs including disability and mental health challenges.
• Person-centred approach and commitment to inclusion guiding interactions with participants and families.

Responsibilities

• Develop personalised support plans within funding budgets.
• Build relationships with service providers and stakeholders to ensure safe service delivery.
• Guide participants on using NDIS plans and provide crisis support when needed.
• Meet reporting requirements to the NDIA to ensure transparency and accountability.

Education

Tertiary qualification in Allied Health discipline
