Job summary

Tender Loving Care Disability Services in Gosford NSW is seeking a dedicated Support Coordinator to ensure seamless coordination of services for clients, helping them achieve goals and develop skills. The role involves engaging with providers, tracking progress, and linking clients to community supports within NDIS guidelines.

The successful candidate will bring strong coordination experience, person-centred practice, trauma-informed approach and excellent communication to support a growing team

Qualifications

• Minimum 2 years of experience in disability or related community services.
• Proven experience in high-level Support Coordination under the NDIS.
• Deep understanding of NDIS processes, price guide, service delivery expectations and relevant legislation.
• Commitment to person-centred, trauma-informed and culturally responsive practices.
• Strong administrative and organisational capabilities including CRM proficiency.
• Excellent interpersonal, advocacy and conflict resolution skills.

Responsibilities

• Coordinate with service providers and allied health professionals to monitor progress toward plan goals.
• Assist customers to identify community or mainstream supports to achieve goals.
• Engage with the community to build meaningful relationships for clients.
• Promote skill development and participation in leisure, sport, and recreational activities.
• Establish and maintain links with the NDIS, TLC programs and other provider services.
• Manage a caseload of complex and potentially at-risk clients with discretion and care.
• Maintain high standards of person-centred practice and compliance with legislation.
