Job summary

Caring Arms Australia is seeking an experienced NDIS Support Coordinator or Specialist Support Coordinator to manage a complex caseload for participants with disability. You will coordinate with health, housing and community services to implement plans and address barriers, driving meaningful outcomes.

Ideal candidates hold relevant social work or allied health qualifications, have strong case-management skills, and can work autonomously within a supportive leadership team.

Qualifications

• Experience in NDIS Support Coordination or Specialist Support Coordination.
• Qualified Social Worker with complex disability or psychosocial support experience.
• Ability to manage own caseload and coordinate across health, housing and community services.

Responsibilities

• Manage a caseload of NDIS participants with complex needs.
• Conduct assessments to identify disability-related support needs.
• Facilitate access to funded, mainstream and community supports.
• Collaborate with families, health professionals and service providers.
• Prepare reports and evidence for plan reassessments.
• Support participants to improve independence and outcomes.
