Job summary

Open Futures Disability Services in Brisbane is seeking a Disability Care Coordinator to coordinate participant supports, lead disability support workers, and ensure services are delivered in line with each participant's needs, goals, and support plans.

The role supports a diverse mix of participants, including those receiving SIL services and those with complex needs, with a focus on safety, quality, and compliance.

Qualifications

  • Demonstrated experience in disability care coordination or a similar coordination role.
  • Care coordination within SIL services, preferably for participants with complex needs.
  • Experience with PBSPs and Restrictive Practices documentation, reporting and escalation.
  • Strong knowledge of NDIS requirements and compliance obligations.
  • Experience supervising or coordinating disability support workers.

Responsibilities

  • Care Coordination: develop, coordinate, and review individualised support with participants and families.
  • SIL Coordination: coordinate SIL services and ensure safe, compliant supports.
  • Complex Support: support participants with complex needs and PBSPs/Restrictive Practices.
  • Compliance & Reporting: document, report, monitor, and escalate incidents per policy.
  • Team Leadership: guide and support disability support workers.
  • Participant Advocacy: maintain person-centred relationships and rights.
  • Documentation: maintain participant records and progress notes.
  • Problem-Solving: address challenges with practical solutions.

Open Futures Disability Servicesis looking for a compassionate, proactive, and experiencedDisability Care Coordinatorto join our team and help ensure our participants receive safe, person-centred, and high-quality support.

About the Role

This is a full-time position offering a competitive salary based on experience. As a Disability Care Coordinator, you will play a key role in coordinating participant supports, leading and supporting our disability support workers, and ensuring services are delivered in line with each participant's needs, goals, and support plans.

The role will support a diverse mix of participants with varying support needs, including participants receiving Supported Independent Living (SIL) services and those with complex support requirements.

Key Responsibilities
  • Care Coordination:Develop, coordinate, and regularly review individualised support arrangements in collaboration with participants, families, support workers, and relevant stakeholders.
  • SIL Coordination:Where required, coordinate SIL services and ensure supports are delivered safely, consistently, and in accordance with participant needs and service requirements.
  • Complex Support:Support participants with complex needs, including those withPositive Behaviour Support Plans (PBSPs)andRestrictive Practices.
  • Compliance & Reporting:Ensure Restrictive Practices and other incidents are appropriately documented, reported, monitored, and escalated in accordance with NDIS requirements and organisational policies.
  • Team Leadership:Provide guidance, supervision, and ongoing support to disability support workers, promoting high standards of care and professional practice.
  • Participant Advocacy:Build strong, respectful relationships with participants and their families, ensuring their preferences, goals, rights, and choices remain at the centre of service delivery.
  • Documentation & Record-Keeping:Maintain accurate participant records, service documentation, progress notes, incident reports, and other required compliance records.
  • Problem-Solving:Respond proactively to participant, staffing, and service-delivery challenges and implement practical solutions.
  • Stakeholder Engagement:Communicate effectively with families, support coordinators, allied health professionals, behaviour practitioners, plan managers, and other relevant stakeholders.
  • Continuous Improvement:Identify opportunities to improve service quality, participant outcomes, team performance, and operational processes.
What We're Looking For

We are seeking someone who is not only compassionate and participant-focused, but who also has the experience and confidence to coordinate complex disability supports.

Essential Experience & Skills
  • Demonstrated experience indisability care coordination or a similar coordination role.
  • Demonstrated Care Coordination experience within SIL services, preferably supporting participants with complex needs.
  • Experience working withPositive Behaviour Support Plans (PBSPs).
  • Practical knowledge and experience relating toRestrictive Practices, including appropriate documentation, reporting, monitoring, and escalation.
  • Strong knowledge ofNDIS requirements, disability support services, and relevant compliance obligations.
  • Experience supervising, coordinating, or supporting disability support workers.
  • Excellent communication and interpersonal skills.
  • Strong organisational, time-management, and problem-solving abilities.
  • Ability to manage multiple participants, priorities, and stakeholders effectively.
  • A person-centred approach and genuine commitment to promoting participant choice, independence, dignity, and quality of life.
  • Strong attention to detail and confidence with documentation and compliance requirements.
