Responsibilities

  • Coordinate day-to-day service delivery, intake, transitions and reviews, keeping individual goals, preferences and safety central.
  • Support and guide support workers and youth workers through clear expectations, effective handovers and regular follow-up.
  • Work with rostering teams to maintain suitable staffing and continuity of care.
  • Maintain accurate case notes, progress updates and incident documentation; follow up outstanding actions and elevate concerns promptly.
  • Collaborate with Child Safety Officers, participants, families, guardians, support coordinators and other stakeholders, respecting consent and information-sharing arrangements.
  • Help teams implement individual support, safety and behaviour plans within approved procedures.
  • Monitor service quality, complaints, risks and feedback, and contribute to practical improvements.
  • Coordinate appointments, transport and placement-related arrangements with relevant teams.
  • Promote culturally safe, trauma-informed and rights-based practice, including meaningful participation by children, young people and people with disability.

Client Regional Coordinator – NDIS & Child Safety

Point Care | Gladstone, Queensland | Full-time

Point Care is seeking a Client Regional Coordinator based in Gladstone to coordinate safe, consistent and person-centred services for people with disability and children and young people in care. This is an operational coordination and team leadership role across NDIS and Child Safety services.

Key responsibilities

  • Coordinate day-to-day service delivery, client intake, transitions and reviews, keeping individual goals, preferences, safety and wellbeing central.
  • Support and guide support workers and youth workers through clear expectations, effective handovers and regular follow-up.
  • Work with rostering teams to maintain suitable staffing, continuity of care and agreed support arrangements.
  • Maintain accurate case notes, progress updates and incident documentation; follow up outstanding actions and elevate concerns promptly.
  • Collaborate with Child Safety Officers, participants, families, guardians, support coordinators, allied health practitioners and other stakeholders, respecting consent and approved information-sharing arrangements.
  • Help teams implement individual support, safety and behaviour support plans within their roles and approved procedures.
  • Monitor service quality, complaints, risks and feedback, and contribute to practical improvements.
  • Coordinate appointments, transport and placement-related arrangements with relevant teams.

Requirements
  • Relevant experience in disability services, child and family services, youth work, community services or a related coordination role.
  • Ability to coordinate competing priorities, support frontline staff and follow through on actions.
  • Understanding of person-centred support, child safeguarding, professional boundaries, confidentiality and trauma-informed practice.
  • Strong written communication, objective incident reporting and accurate record-keeping skills.
  • Ability to build respectful relationships with participants, young people, families and professional stakeholders.
  • Confidence using digital case-management, rostering and reporting systems.
  • Ability to work from Gladstone and meet the role’s local service-delivery needs.
  • Relevant qualifications in community services, disability, youth work, social work, nursing or a related field are desirable.
