Job summary

Complete Nursing & Home Care is seeking a Service Coordinator to assess, schedule and monitor services for clients, enabling independence and community connection. You will work with clients and families, maintain schedules, and ensure staff qualifications and compliance with NDIS standards.

The role involves rostering, personal and clinical support coordination, and collaboration with the Senior Administrator Officer to manage service agreements. A supportive team environment is offered.

Responsibilities

  • Schedule care and support including personal care and clinical support.
  • Coordinate with customers and communities to meet needs.
  • Manage consumer directed care queries with flexible coordination.
  • Maintain participant and staff information in scheduling/CRM systems.
  • Match carers and services using a strengths-based approach.
  • Ensure workers have required qualifications and checks.
  • Collaborate with Senior Administrator Officer on service agreements.
  • Support compliance with NDIS Practice Standards and policies.
  • Deliver services per company policies and procedures.
  • Lead response to incident reports, complaints, safeguarding and quality concerns.
  • Demonstrate diversity awareness and ongoing professional development.
  • Communicate effectively with customers, staff and allied health professionals.
  • Understand work risks and mitigation strategies.
  • Assist with payroll and timesheet reconciliation.
  • Other tasks aligned with skills and training.

CNHC’s Service Coordinator is the primary point of contact for all our clients and is responsible for assessing, scheduling, and monitoring services to clients and providing individualised care to enable our clients maintain independence and connection to the community.

Desirable Qualifications

Certificate in Disability/Aged Care or Community work is desirable

Relevant qualification in the disability sector

Prior rostering experience

Extensive knowledge of NDIS and Practice Standards

Experience working in the welfare/community services sector, particularly in disability

Experience or knowledge of current My Aged Care policies

Ongoing Duties and Responsibilities

To work with individuals and their families to schedule care and support including personal care, clinical support and engaging with customers and communities.

To manage consumer directed care queries flexible and tailored coordination to meet specific customers' needs and incorporate shared care planning.

Ability to work with company’s CRM rostering system. Maintain participant and staff information within scheduling and CRM systems.

To use strengths-based approach together with a strong collaboration approach to assessing and matching carers and services in the areas of need where the customer and their immediate supports cannot provide.

To ensure workers allocated to shifts possess required qualifications, screening checks, and competencies.

To work closely with Senior Administrator Officer for service agreements.

Ability to support compliance with NDIS Practice Standards and organisational policies.

To ensure the services are delivered in line with the company's policies and procedures.

To lead immediate response to Incident Reports, follow up, and management. Receive, acknowledge, accurately document and escalation complaints, concern safeguarding issues, incidents or service quality concerns to the manager within required timeframes.

To demonstrate awareness of diversity and be respectful to all stakeholders of the enterprise.

To demonstrate a commitment to ongoing professional development.

To demonstrate effective communication skills with customers, staff, and allied health professionals.

To understand the risks associated with work and ways to mitigate these.

To assist with payroll and timesheet reconciliation processes.

Other related tasks consistent with your skills and training.
