$80,000 + superannuation + on-call allowance

As a Service Coordinator, you'll report to the Service Manager and work closely with our Care Coordinator and wider Service Management Team. You'll be responsible for coordinating day-to-day services and ensuring participants, families, support workers and stakeholders remain connected and informed. Every day brings something different. You'll play an important role in supporting our participants and team by coordinating rosters, helping with last-minute changes, welcoming new participants, keeping compliance on track, and making sure everyone has the support they need. You'll be part of a friendly, supportive team where your organisation, communication and care can make a real difference every day.
Build and manage support worker rosters
Coordinate last-minute shift changes, cancellations and coverage
Match support workers with participants based on skills, experience, training and participant preferences
Coordinate the onboarding of new participants, including service agreements and schedules of support
Communicate regularly with participants, families, Support Coordinators, Plan Managers and allied health professionals
Monitor shift notes, progress notes and timesheets for accuracy and completeness
Report, coordinate and follow up incidents and complaints in accordance with NDIS requirements
Assist with worker compliance, including screening, training and competency records
Maintain accurate documentation and support audit readiness
Help identify and resolve service delivery issues quickly and professionally
Participate in the on-call roster, with an on-call allowance provided
