Job summary

Triniti Home Care Pty Ltd in Campbelltown is seeking an experienced Service Coordinator to join our team. You will be responsible for coordinating day-to-day services, rostering, and ensuring participants and workers stay connected and informed.

You'll work with a nurse-led leadership team to ensure the right support at the right time, manage on-call duties, and help maintain compliance and documentation across the service management team.

Qualifications

• NDIS experience required.
• Experience coordinating rosters and services.
• Strong communication with participants and families.

Responsibilities

• Build and manage support worker rosters.
• Coordinate last-minute shift changes, cancellations and coverage.
• Match support workers with participants based on skills and preferences.
• Coordinate onboarding of new participants, including service agreements and schedules.
• Communicate regularly with participants, families and care teams.
• Monitor notes and timesheets for accuracy and completeness.
• Report and follow up on incidents and complaints in line with NDIS requirements.
• Assist with worker compliance and training records.
• Maintain accurate documentation and audit readiness.
• Identify and resolve service delivery issues promptly.
• Participate in the on-call roster with an on-call allowance.
