Job summary

Qualifications

• Experience providing case management, outreach and service coordination.
• Strong person-centred and strengths-based practice.
• Excellent communication and stakeholder engagement skills.
• Experience completing assessments, referrals, case notes and client documentation.
• Understanding of privacy and confidentiality obligations.
• Relevant qualifications in Social Work, Community Services or related field.

Responsibilities

• Provide case management, advocacy and practical support to children, young people and families.
• Build strong relationships and support service users to achieve their goals.
• Conduct assessments, referrals and service coordination.
• Work with government and community service providers.
• Maintain accurate case notes and reporting requirements.
