Job summary

Warrigal Home Services is seeking a dedicated Care Partner to coordinate and manage client care in the Southern Highlands region. You will balance multiple caseloads while ensuring services align with individual care plans and approved frameworks.

You will collaborate with clients, families, service providers and internal teams to deliver safe, efficient care, focusing on outcomes and wellbeing. A Diploma or Certificate IV in related fields is required.

Qualifications

• Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent.
• Demonstrated experience in care coordination, case management or related roles in aged, health or disability sectors.
• Advanced assessment and care planning skills with ability to identify client needs, risks and support requirements.
• Experience in multidisciplinary care environments and the ability to network, negotiate and advocate.

Responsibilities

• Coordinate and manage care needs of community clients to align with care plans and approved frameworks.
• Undertake comprehensive assessments and develop/review care plans.
• Collaborate with clients, families, service providers and internal teams to ensure safe, efficient service delivery and positive client outcomes.
