Job summary

Healthcare Australia is seeking a Home Care Coordinator to support clients across the Armidale area. You will meet clients in their homes, assess needs, develop care plans, and work with internal teams to deliver outstanding service.

You’ll spend time visiting clients and coordinating care with nurses and allied health professionals. You are organised, proactive, and adept at managing multiple priorities while maintaining documentation and compliance.

Qualifications

• Experience in Home Care, aged care, community services, healthcare, care coordination or case management.
• Strong communications, relationship-building and customer service.
• Ability to manage multiple priorities and maintain accurate documentation.

Responsibilities

• Monitor service delivery through regular communication, care plan reviews, and client feedback to ensure satisfaction and high standards.
• Collaborate with Registered Nurses and Allied Health professionals to coordinate care for clients with complex needs.
• Work with internal teams to ensure services are delivered efficiently and meet client expectations.
