HomePlace seeks a Senior Coordinator to oversee safe, high-quality, person-centred supports, helping participants achieve their goals and live meaningful lives. This permanent full-time role leads the design and delivery of individualised supports and mentors Senior Support Workers to ensure outcomes and quality of life.

The role requires strong leadership, collaboration with stakeholders, and a commitment to safeguarding and continuous improvement in a dynamic community services environment.

Qualifikationen

• Degree or equivalent leadership experience in disability/community services
• Minimum three years leading disability or human services teams
• Experience managing Supported Independent Living or similar services
• Strong communication and relationship-building skills
• Knowledge of NDIS Practice Standards and safeguarding

• Lead design, implementation and review of individualised supports
• Ensure participants are involved in decisions about their lives and supports
• Oversee service delivery to promote independence and self-determination
• Monitor outcomes and quality of life indicators
• Lead complex decision-making and service planning
• Coach and develop Senior Support Workers
• Ensure safeguarding and risk management compliance
• Build relationships with participants, families and stakeholders
• Drive continuous improvement and innovative approaches
