Job summary

SA Metro Care is seeking an experienced Service Coordinator to join our Adelaide team. This full-time role leads the day-to-day management of an allocated portfolio of participants, coordinating supports and ensuring person-centred services within a compliant framework.

You will supervise Support Workers, maintain essential participant documentation, and collaborate with families, guardians and health professionals to deliver safe, responsive supports aligned with each person's goals and needs.

Qualifications

Diploma or Certificate IV in Community Services (mandatory).
Bachelor’s degree in Community Services or related discipline (desirable).
Experience working in disability services with complex support needs.
Strong knowledge of NDIS Practice Standards and Code of Conduct.
Experience coordinating a participant caseload and documentation.
Excellent written and verbal communication and organisational skills.

Responsibilities

Manage day-to-day service delivery for an allocated portfolio of participants.
Supervise Support Workers to ensure safe, person-centred service delivery.
Maintain participant documentation, including Support Plans and Risk Assessments.
Conduct regular participant reviews and update supports accordingly.
Support incident management and safeguarding processes with stakeholders.
Liaise with Behaviour Support Practitioners, allied health professionals and families.
Ensure compliance with NDIS requirements and organisational policies.
