Qualifications

• Minimum 2 years experience working with people with intellectual or other disability and their families.
• Experience coaching or supporting support staff in community settings.
• Experience supporting people with complex health care issues.
• Current NDIS Clearance and Working With Children Check.
• Hold a current driver's licence and access to a vehicle.

Responsibilities

• Provide high-quality direct support while overseeing supports within the person’s home.
• Contribute to support planning, goal development and monitoring participant outcomes.
• Support the implementation of behaviour support, communication and health care plans.
• Facilitate community connections, meaningful relationships and opportunities for social participation.
• Mentor, coach and support team members to deliver consistent, high-quality support.
• Oversee day-to-day household operations, including budgets, resources, maintenance and safety requirements.
• Build positive relationships with families, representatives, health professionals and service providers.
• Advocate for the rights, choices and aspirations of the people HomePlace supports.
