Job summary

Ozcare is seeking a compassionate Care Partner to support older clients in Mackay Community Care. The role starts part-time with the view to move to full-time within 6 months, focusing on independent living and high-quality care.

You will plan, manage and lead client care across programs, coordinating multi-disciplinary teams, services and families to deliver holistic care. A health-related qualification and valid certifications are required, with flexible start times and opportunities for

Qualifications

• Experience in case management and client care coordination in Community Aged Care.
• Strong verbal, written and interpersonal communication with diverse groups.
• Vaccination against influenza and Covid-19 (or willingness to vaccinate).
• Current Police Screening Certificate.
• Current First Aid and CPR Certificate.

Responsibilities

• Manage and coordinate care services to ensure high standards of care.
• Develop and implement tailored care plans for clients.
• Liaise with clients, families and healthcare professionals.
• Build strong relationships with clients and team members; support clients’ goals.
• Ensure compliance with legislation and organizational policies.
• Monitor and evaluate care services to improve quality and efficiency.
• Monitor Home Care Package budgets and explain funding/fees to clients.
