Job summary

Ozcare is seeking a compassionate Care Partner in Mackay Community Care, with a view to moving from part-time to full-time within six months. You will plan, manage and lead client care across programs, coordinating multi-disciplinary teams to support independent living for older adults.

Responsibilities include developing tailored care plans, liaising with clients and families, ensuring high standards of care, and monitoring quality and compliance.

Qualifications

• Experience in case management and client care coordination in community aged care settings.
• Strong verbal and written communication with diverse stakeholders.
• Vaccination against influenza and Covid-19 as required, and immunisation proof for MMR/Varicella/Pertussis.
• Current Police Screening Certificate and First Aid/CPR certifications.

Responsibilities

• Plan, manage and lead client care and support across programs.
• Coordinate involvement of multi-disciplinary teams and services.
• Develop and implement tailored care plans for individual clients.
• Coordinate with clients, families and healthcare professionals to ensure comprehensive care.
• Monitor care quality, compliance with policies, and budget understanding.
