Ozcare is seeking a compassionate Care Partner to support older clients in Mackay Community Care. The role starts part-time with the view to move to full-time within 6 months, focusing on independent living and high-quality care.

You will plan, manage and lead client care across programs, coordinating multi-disciplinary teams, services and families to deliver holistic care. A health-related qualification and valid certifications are required, with flexible start times and opportunities for

Qualifications

• Experience in case management and client care coordination in Community Aged Care.
• Strong verbal, written and interpersonal communication with diverse groups.
• Vaccination against influenza and Covid-19 (or willingness to vaccinate).
• Current Police Screening Certificate.
• Current First Aid and CPR Certificate.

Job Description

Ozcare has a Part-time Care Partner position available at the Mackay Community Care branch, with the view to move to Full time within the next 6 month. We are looking for a compassionate individual who can help support our older generation in living independently.

Your Role

You’ll be responsible for planning, managing and leading client care and support across a range of programs. You’ll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.

As a Care Partner you will

• Manage and coordinate care services to ensure the highest standards of care
• Develop and implement care plans tailored to individual client needs
• Liaise with clients, families, and healthcare professionals to ensure comprehensive care
• Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
• Ensure compliance with all relevant legislation and organizational policies
• Monitor and evaluate care services to continuously improve quality and efficiency
• Monitor the budget of Home Care Packages and ensure clients understand their funding and fees

For Success you need to

• Demonstrate experience in case management and client care coordination in a Community Aged Care setting
• Have demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
• Be vaccinated against Influenza & at least 1 dose of Covid-19 vaccine
• Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
• Hold a current Police Screening Certificate
• Provide a copy of your current First Aid and CPR Certificate
• Hold a current Queensland driver's licence
• Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
