Job summary

CoAbility is seeking an experienced Support Coordinator to manage a Level 2 and Level 3 caseload from Melbourne’s South East. The role is primarily work from home with occasional face-to-face meetings, based around Oakleigh in the south-east of Melbourne.

You will turn plans into progress, keeping participants in the driver’s seat while building capacity, and connecting with providers to negotiate service agreements. A strong NDIS background and affirming practice are essential.

Qualifications

• At least 2 years' experience delivering NDIS Support Coordination.
• Tertiary qualification in Social Work, Community Services, Allied Health or related field.
• Current NDIS Worker Screening Check and Working with Children Check.
• A valid Victorian Driver's Licence and access to a comprehensively insured vehicle.
• Private home office with reliable internet, own laptop and mobile phone (BYOD allowance).

Responsibilities

• Turn plans into progress: lead complex plans so funding becomes real-life progress.
• Ensure participants stay in the driver’s seat with clear information to exercise control.
• Build participants' skills and confidence to manage supports independently over time.
• Connect participants with high-quality providers and negotiate service agreements.
• Write progress reports showing goal attainment to support funding decisions.
