Job summary

Hope and Change is seeking an experienced NDIS Support Coordinator to join our growing team in a full-time, hybrid role spanning home, Indooroopilly office and community settings.

You will work autonomously to help participants understand and implement their NDIS plans, while collaborating with an experienced team that values accountability and quality practice. The role involves managing caseloads, building relationships and ensuring seamless supports tailored to individual needs.

Qualifications

• Minimum one year of experience in NDIS Support Coordination.
• Strong working knowledge of the NDIS and plan implementation.
• Ability to manage a participant caseload independently.
• Ability to interpret NDIS plans, budgets and funding.
• Strong written communication and report-writing skills.

Responsibilities

• Develop strong professional relationships with participants and their support networks.
• Help participants understand their NDIS plans, funding and supports.
• Connect participants with disability, health, housing, community and mainstream services.
• Source and coordinate providers based on participant needs and goals.
• Monitor plan utilisation, budgets and funding risks.
• Prepare reports and evidence for plan reviews and reassessments.
• Maintain accurate case notes and participant records.
• Communicate with families, nominees, providers, clinicians and stakeholders.
• Identify risks and escalate as needed.
• Manage caseload, workflow and priorities independently.
