Danny Met Sally is seeking a dedicated NDIS Support Coordinator to join our team and deliver premium services to Participants.

This full-time role is remote/working from home and can be based anywhere in Brisbane or the surrounding region, with ongoing case management.

You will build rapport, support plan implementation, coordinate services, advocate for participants, and collaborate with health, housing and government partners.

Qualifikationen

• Minimum 2 years as a Support Coordinator or Specialist Support Coordinator.
• Tertiary qualification in Social Work, Human Services, Community Services, Psychology, Disability, Mental Health, or related field.
• NDIS Worker Screening Check (Yellow Card) and Working with Children Check.
• Drivers’ License and own reliable transport.
• Experience supporting participants with complex needs and navigating NDIS and community systems.
• Proven ability to work with multidisciplinary teams and manage risk.

• Build rapport and maintain relationships with Participants.
• Support participants to understand and implement their NDIS plans.
• Coordinate and monitor supports to achieve goals.
• Advocate for participants for services, housing, health and community supports.
• Prepare reports and evidence for NDIS plan reassessments.
• Facilitate stakeholder meetings and collaborative planning.
• Maintain records following NDIS standards and privacy rules.
• Manage a diverse caseload and priorities.
