Job summary

Healing Care Support Services in Melbourne is seeking an experienced NDIS Support Coordinator to join as a contractor. You will manage a varied caseload, support participants with complex needs, and work closely with the Director to deliver person-centred outcomes.

This primarily remote role offers about 15–25 hours per week to start, with potential to grow as the business expands. You will attend face-to-face visits across Melbourne when required and collaborate with families, carers and

Qualifications

• Minimum 12 months' NDIS Support Coordination experience.
• Qualifications in Social Work, Community Services, Allied Health, Disability or related field.
• Current NDIS Worker Screening Check and Working with Children Check.
• Ability to work remotely and attend face-to-face visits across Melbourne when required.

Responsibilities

• Support participants to understand and implement their NDIS plans.
• Help participants exercise choice and control.
• Build strong, respectful relationships with participants, families and carers.
• Respond promptly and professionally to participants, families, providers and care team members.
• Follow up participant needs, concerns and outstanding actions in a timely manner.
• Connect participants with suitable and high-quality service providers.
• Liaise with allied health professionals, support providers, plan managers and other stakeholders.
• Monitor NDIS funding, plan utilisation, goals and support needs.
• Review and negotiate service agreements.
• Identify gaps in supports and respond proactively to issues.
• Prepare progress reports and documentation for NDIA reassessments and reviews.
• Maintain clear, accurate and timely case notes.
