Job summary

Keep Connected Support Coordination is expanding and seeking a passionate, hard-working person to join the team. This is a strictly work from home role, with occasional travel to meet participants and stakeholders.

We are a small independent provider in South East Melbourne, offering support coordination and specialist coordination. Ideal candidates will have tertiary education in disability services or substantial industry experience, NDIS clearance, and a drivers license.

Qualifications

• Tertiary education in Disability, Community Services, Social Work, Psychology or substantial industry experience.
• NDIS Worker Screening Clearance
• Current Drivers License
• Working with Children’s Check
• Previous experience as a support coordinator or in a case management role
• Strong knowledge of the disability and community services sector
• Experience working with people with disabilities, child protection involvement, homelessness, Alcohol and Other Drugs and mental health
• NDIS Online Orientation Module

Responsibilities

• Assist clients to exercise choice and control by understanding and implementing their NDIS plans
• Support clients to connect to NDIS and mainstream services
• Support participants through planning meetings and reviews
• Adhere to thorough case note recording and prepare NDIS reports as required in line with support coordination requirements
• Meet weekly KPI targets set out by Keep Connected
• Work autonomously with strong organizational skills and self motivation
