Job summary

Calvary Home Care - Adelaide is seeking an experienced registered nurse to deliver overnight palliative care to clients at home or in residential settings. You will provide complex symptom management, high-quality patient education and compassionate end-of-life support.

You will work independently within a supportive clinical team, maintain thorough documentation, and collaborate with families, carers and referrers to ensure safe, person-centered care while upholding Calvary's mission and values.

Qualifications

• Current AHPRA Nursing registration.
• Minimum 2 years palliative care nursing experience; community experience desirable.
• Postgraduate qualifications in Palliative Care (or working towards).
• NDIS Clearance (or willingness to obtain one).
• Police Clearance within 6 months (or willingness to obtain one).
• Use of own car; KM allowance provided.
• Full driver's licence and comprehensively insured car.
• CPR and Manual handling.
• Commitment to joining a mission-based organisation.

Responsibilities

• Provide palliative care to clients in home or residential settings.
• Educate and support families and carers.
• Administer and prepare subcutaneous medications and CSIs for symptom management.
• Perform general nursing care including wound care and IDC/SPC care, PICC dressings.
• Accurate documentation in client records.
• Communicate with clinical teams, community palliative care teams and referrers.

About the role

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client's home or residential accommodation.

Key Responsibilities

• Support and education for clients with terminal diagnoses
• Education and support for families and carers
• Administration and preparation of s/c medications and CSCI for symptom management
• General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
• Accurate/ Timely documentation in all client records.
• Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
• Complete initial assessments/develop care plans to ensure best outcome for our clients.
• Monitor and update care plans to ensure appropriate care is provided
• Recognise and respond to clinical deterioration, with appropriate escalation.
• Promoting best practice and initiating quality improvement activities
