Job summary

Hall & Prior Health & Aged Care Group in Western Australia is seeking a passionate and experienced Support Coordinator to join our Disability Services team on a part-time basis, four days a week. You will work with NDIS participants to build capacity, support plans, and navigate funding across informal and funded supports, helping clients achieve their goals while maintaining choice and control.

The role requires strong communication and the ability to manage a caseload; we offer flexible work

Qualifications

• Experience in NDIS Support Coordination or related disability services.
• Understanding of the NDIS framework and service pathways.
• Excellent communication, relationship-building and problem-solving skills.
• Ability to manage a caseload and work autonomously.
• Current checks: NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).

Responsibilities

• Coordinate with NDIS participants to build capacity and independence.
• Navigate and maximise participants' NDIS plans and supports.
• Provide coordination across informal, community and funded supports to achieve goals.
• Maintain participant choice and control while delivering outcomes.
